Assumption of Risk

I understand that participating in activities at John Hay Sports Pavilion (including basketball, pickleball, and other physical activities) involves risks of injury, illness, property loss, and even death. I voluntarily accept full responsibility for these risks.

Release of Liability

I release and hold harmless John Hay Sports Pavilion, its owners, staff, and affiliates from any and all claims or liabilities arising from my use of the facility, including injuries, loss, damage, or death, whether caused by negligence or otherwise.

Personal Belongings

I acknowledge that John Hay Sports Pavilion is not responsible for any lost, stolen, or damaged personal property while on the premises.

Medical Consent

In the event of an emergency, I authorize John Hay Sports Pavilion to obtain medical treatment for me. I accept full responsibility for any resulting expenses.

Rules & Conduct

I agree to follow all rules and directions provided by staff. I understand that failure to comply may result in being denied access without refund.

Photo/Video Consent

I consent to the use of any photo or video taken during my visit for marketing or promotional purposes.

Legal Agreement

I confirm that I am at least 18 years old or have the consent of a parent or legal guardian. This agreement is governed by the laws of the Republic of the Philippines.
